The app gives you control over more than just the fields displayed on a form. You can customize the form’s appearance, configure what happens after submission, set up automated email responders, and, on supported plans, send submission data to CRMs, marketing platforms, and external APIs.

1. Form Creation and Form Builder
Create a form by choosing from 30+ predefined templates based on available categories or by starting with a blank form in the Form Builder.
The Form Builder supports multiple input types and gives you control over the visual presentation of the form. You can customize colors and themes, adjust form appearances and styles, add background images, and configure other available UI options.
This allows you to create forms that match the purpose and design of your Shopify store instead of relying on a fixed default layout.
3. Form Draft and Submission Flow Configuration
New forms are saved as Draft by default, allowing you to configure the form before making it available to customers.
You can also define what should happen after a customer submits the form. Depending on your configuration, the form can:
- Redirect the customer to a specific URL
- Clear the form
- Display a customized thank-you message
This lets you control the post-submission experience based on the purpose of the form.
4. SMTP and Email Responder Configuration
You can configure an SMTP connection to send automated emails from your forms.
The app supports separate configurations for:
- Admin Responder – sends notifications to your team or other specified recipients.
- User Responder – sends an email response to the person who submitted the form.
Admin notifications can also be sent to multiple email recipients, making it possible to notify the appropriate people when a new submission arrives.
5. Email Templates and Responders
Email responders are disabled by default and can be enabled when needed.
When configuring a responder, you can select an existing SMTP connection or create a new one. The app also supports predefined email templates as well as customized templates for both Admin and User responders.
This allows you to control the emails sent after a form submission rather than relying on a single fixed message.
6. CRM and API Integration
CRM and API integrations allow submitted form data to be sent to connected external systems.
This functionality is available to Premium subscribers. Free or lower-tier users can create and publish forms but do not have access to CRM/API integration capabilities.

A single form can be connected to multiple supported CRM and marketing platforms.

10. OAuth 2.0 Authentication
When a supported service uses OAuth 2.0, you can establish the CRM connection using the required OAuth credentials and authentication settings, such as secret keys, rotating tokens, and other required configuration values.
After the connection is established, you can test and validate it before making it available for use with your forms.
11. Reusable CRM Connections
Once a CRM or API connection has been configured and successfully tested, you can reuse it across forms.
Instead of entering the API endpoint and authentication details again, you can select an existing connection from a dropdown while configuring a form.
This reduces repeated setup when multiple forms need to use the same external system.
12. Multiple CRM Connections Per Form
A single form can be associated with multiple CRM connections.
Each submission can then be processed according to the CRM integration and mapping rules configured for those connections. This makes it possible to use one form as the source for multiple external workflows.
13. Form Publishing and Shopify Embed
After your form, responder, and CRM configurations are complete, you can publish the form.
When the form is published, the app generates Script/HTML embed code with predefined properties. You can copy this code for use with your Shopify store.
14. Shopify Store Integration
To display the published form on your storefront, copy the generated Script/HTML tag and add it to the appropriate Liquid/HTML configuration area in your Shopify store.
Once the generated script is added, the form can be rendered on the selected Shopify store page.
15. Form Submission Management
When a customer or lead submits a form, the submission is immediately stored in the app’s dedicated Submission Records table.
The same submission can also be sent to configured CRM systems according to the integration settings associated with the form.
This gives you a record of collected submissions while also allowing the information to move into connected systems.
16. CRM Integration Logs
The app maintains dedicated integration logs for each CRM connection.

17. Premium Subscription Restriction
CRM and API integration features are available exclusively under the Premium subscription plan.

18. Advanced Form Elements
The Form Builder now includes eight advanced form elements for collecting more detailed and interactive information: Switch, Range Slider, Repeater, Digital Signature, Matrix Grid, Star Rating, Color Picker, and Color Swatch.
Use these fields to collect things like preferences, numerical ranges, multiple entries, signatures, ratings, and color selections without needing additional form apps. These elements also support validation, styling, and conditional logic.
19. Custom Styling
Custom Styling lets you adjust the appearance of your forms to better match your Shopify store. You can customize colors, typography, spacing, button styles, borders, and other visual elements.
Custom CSS is scoped to the form container, helping prevent your form styles from affecting other parts of your Shopify theme. CSS is also sanitized before it is saved.
20. Multi-Step Forms
Multi-Step Forms let you split longer forms into separate pages or steps instead of displaying every field on one screen.
For example, you can use one step for contact details, another for project requirements, and a final step for review and submission. Customers can see their progress as they move through the form, and required fields are validated before they continue to the next step.
21. Smart Conditional Fields
Conditional Fields let you create forms that respond to customer selections. Fields can automatically appear or stay hidden based on the information a customer provides.
For example, selecting Technical Support can display a Product Serial Number field, while selecting another inquiry type can keep it hidden. This helps you show customers only the questions that are relevant to their situation.
22. Duplicate, Export, and Import Forms
Managing multiple forms is easier with the new Duplicate, Export, and Import options.
You can duplicate an existing form to create a new version without building it from scratch, export forms as .json files for backups or transfer, and import saved templates when you need to restore or reuse a form. Sensitive credentials are removed from exported templates before the file is created.
23. Fast Search and Pagination
The Forms and Submissions areas now use server-side pagination and optimized search to keep the admin experience responsive as your form and submission data grows.
Instead of loading every record at once, the app loads records in manageable pages. You can also search by keywords, email addresses, date ranges, or form IDs to find the information you need more quickly.
How the Shopify App Works
Every form follows the same basic workflow:
Form → Submission → Email / API / CRM
The exact steps depend on how you configure the form, but the overall process is straightforward.
1. Build Your Form
Start by creating a form in the app dashboard.
You can select a predefined template or create a form from scratch using the Form Builder. Add the fields you need, choose the appropriate input types, and customize the form’s appearance and styling.
Once created, the form remains in Draft status until you are ready to publish it.
2. Publish the Form to Your Shopify Store
Once the form configuration is complete, publish it.
The app generates Script/HTML embed code that you can copy and add to the appropriate Liquid/HTML configuration area of your Shopify store. The form is then rendered on the selected storefront page.
3. A Customer Submits the Form
When a customer or lead completes the form and submits it, the submission is stored in the app’s Submission dashboard.
This creates a record of the submitted information that you can manage within the app.
5. The App Processes the Submission
After submission, the app follows the actions configured for that form.
Depending on your setup, the submission can:
- Send an email notification
An Admin Responder can notify one or more recipients, while a User Responder can send an automated response to the person who submitted the form.
- Send data to a CRM or external API
On Premium plans, the submission can be sent to connected CRM, marketing, or external API systems according to the configured connection and field-mapping rules.
6. Review Integration Activity
For CRM connections, the app maintains integration logs that record requests, responses, execution status, success or failure information, and integration errors.
These logs can be used to investigate synchronization problems and understand what happened when a submission was sent to an external system.
